INSURANCE PROGRAM - Claim Forms Personal Accident Claim – Non Medicare Medical Expense To submit a claim for Non Medicare Medical Expenses and/or Loss of Income, please download the claim form by clicking on the document below. Complete this form as fully as possible and have your centre sign and return to NSW Squash along with any medical receipts for reimbursement. They will forward it to Horsell International on your behalf.
If there appears to be a potential capital benefits claim for a permanent of partial impairment, please notify Horsell International immediately.
