Personal Accident Claim – Non Medicare Medical Expense & Loss of Income

To submit a claim for Non Medicare Medical Expenses and/or Loss of Income, please download the claim form by clicking on the document below. Complete this form as fully as possible and have your club sign and return to the Australian Karate Federation. They will forward it to Horsell International on your behalf.

Personal Accident Capital Benefit Claim

If there appears to be a potential capital benefits claim for a permanent of partial impairment, please notify Horsell International immediately. Public Liability Claims

In the event of a public liability claim you must notify Horsell International immediately and do not admit liability or discuss the matter with the third party.
