Claim Notifications

Please go to the Client Area and click on your client logo or name to access claim forms and other documents applicable to you and your organisation.

Organisation-specific documentation is currently available via the Client Area for:

• Australian Karate Federation
• Australian Little Athletics
• Basketball Australia
• NSW Squash
• Petanque Australia
• Soccer NSW Futsal
• Squash Brisbane

Urgent Claim Notifications

If your organisation name does not appear in the Client area and you have an urgent claim notification, please contact Horsell International on +61 2 9247 1700 (24 hours) or within Australia 1300 722 990 (24 hours, local call cost).

Other Claim Notifications

If your organisation name does not appear in the Client area and you have a non-urgent claim notification, please provide Horsell with the information listed below. Once your details have been received, a Horsell representative will contact you to process your claim. Please ensure you provide Horsell with as much detail as possible regarding the loss/accident/incident.

Required Information
The following information MUST be included when sending a Claim Notification:

1. Name
2. Email address
3. Telephone number (including country and area code)
4. Nature of loss/accident/incident
