Basketball Australia Compulsory Cover Claim Form Personal Accident Claim - Non Medicare Medical Expense & Loss of Income To submit a claim for an injury sustained between 1st September 2008 4pm Local Standard Time and 1st September 2009, 4pm Local Standard Time, please download the claim form by selecting the relevant state from the drop down list and clicking the Download button. Complete the claim form as per the instructions provided. If your injury was sustained after 4pm Local Standard Time 1st September 2009, please contact Basketball Australia or your State Association for the appropriate claim form and details of how to make a claim.
If there appears to be a potential capital benefits claim for a permanent of partial impairment, please notify Horsell International immediately.
